Fees
Individual, Couples, and Family Counseling: $100. for the first one hour session and $85. for each subsequent session.
Classes: $15. for each 1 hour class (including materials). The total amount for all six classes is to be paid in advance ($90.)
Insurance Accepted
Now taking Aetna, Lovelace, Presbyterian, ValueOptions, United, Optum Health, Medicaid, and BCBS. I am an out-of-network provider for most other insurances.

Informed Consent
When agreeing to treatment with the use of insurance/managed care for payment it is assumed you understand the following:
1) Insurance companies and/or managed care companies have the right to review all treatment plans, progress notes, and any other information in the chart in order to determine payment, quality of care, and/or compliance with records keeping procedures.
2) Insurance companies and/or managed care companies may determine how many sessions you are allowed and in what time frame you may use those sessions.
3) A psychiatric diagnosis must be provided to qualify for third party benefits. That diagnosis becomes part of your permanent medical record.
4) Some managed care companies require that I, as the mental health provider, discuss with them over the phone your situation, diagnosis, reasons for needing treatment, and treatment goals.
